Sunday School Lesson

Today our Sunday School lesson was Matthew 11-12, Luke 11.  Talking about coming unto Jesus and being yoked with Him.  We also talked about the Sabbath day, honoring it and keeping it holy.

Lisa taught the lesson today.  I made the handouts as per instructed.  :-) Lisa put together cute little baggies with Easter candy and the handout.  Easter will be before our next lesson because we won't be teaching first Sunday in April due to general conference.

Lisa always does a good job.

Here are the handouts with her words and photo of the finished handouts.

Falsified Medical Record

I'm really irked right now with humanity.  Namely so-called medical professionals.  This morning Lisa took me to an eye appointment.  It's been almost 4 years since I've had a diabetic eye exam and I need a new prescription for eyeglasses.  I called around to many optician offices and no one will see me because they claim they can't accommodate my wheelchair and my not being able to be transferred.

So I made an appointment with Cascade eye and skin – which is the place I went to only a few days before dad died– which was my last eye exam.  I remember it well.  They were able to treat me then and gave me my exam.  So, even though they no longer take my insurance, I was willing to pay out of pocket to go back to get my eye exam.

Well, to make a long story short, they wouldn't treat me because I can't wear a mask.  I explained that I can't drive my wheelchair with the mask on.  So they told me I had to wear a mask while I'm sitting there.  I told them I have a hard time breathing because I only have use of my diaphragm and not intercostal or abdominal muscles to assist me in breathing.  (I was already having a hard time breathing today). They didn't care.  They wanted to murder me by suffocation.  So,I told her I can't and we will just have to leave.  So, we just left the waiting room and came home.

But, that's not what this blog is about.  Although, coincidentally, it's in the same office building as the gastroenterology appointment I had which is what this blog is about.

I had a useless gastroenterology follow-up on Monday.  The PA – Shayla Fawcett – diagnosed me with and treated me empirically for small intestinal bacterial overgrowth (SIBO) on December 16, 2022. Which incidentally the treatment was antibiotics (Augmentin) which made my condition much worse.  So it turns out, that the diagnosis of SIBO was wrong. And I'm still suffering the repercussions of that misdiagnosis every day since I took the antibiotics in January.

Okay, so fine, there's nothing they can do to help me.  I had zero intention of going back for another useless follow-up.  Which I told the scheduler when she called to schedule another follow-up.  But what irks me is the falsifications on my soap notes (medical records).

The funny thing is, I probably never would've even seen the soap note from my March 13, 2023 follow-up if they hadn't sent me an email telling me I had a message to read.  So I logged onto my gastroenterology my chart and read my soap note.

Then I got mad because it was full of lies.

There was no physical exam performed.  There was no breath test for SIBO performed ever.  And, she did not discuss with me any risk factors for metoclopramide except tardive dyskinesia.  Even after I specifically asked her for other risk factors.

Here are the excerpts from my soap note and my communication with their office via my patient portal messaging system on their website.

I have a problem with the soap note that Shayla wrote for my follow-up on March 13, 2023. In the note it states that a breath test was done for sibo which came back negative. No such test was done.

It also states that a physical exam was performed. She did not perform a physical exam on me. She did not come close enough to look at me let alone touch me to perform a physical exam. It also states ambulatory under physical exam. Which is incorrect.

And, the note also states that risk factors for metoclopramide were discussed. The only risk factor she mentioned was tardive dyskinesia. Even after I specifically asked her what the other risk factors were, she reiterated just tardive dyskinesia. So the other risk factors mentioned in the soap note were never discussed during my visit.

I am not happy with these falsifications in my medical records.

Sincerely,

I sincerely apologize for the mistyped portions of the visit. To clarify regarding the concerns:
The physical exam documented is all visual and based on communication (no abdominal exam was done this visit or documented, nor were other portions that required me to touch the patient). I did by mistake document ambulatory in the initial functional/cognitive portion, however in the musculoskeletal portion it is accurately documented that she is in a wheelchair and has quadriplegia.
The mistyped ambulatory portion was corrected in an addendum. I also corrected the mistyped portion in the diagnosis comments regarding Breath Test for SIBO. She was treated empirically for SIBO at previous visit rather than order Breath Test at that time, as I discussed with her in clinic SIBO less likely as she did not respond well to empiric treatment with Augmentin. We did discuss tardive dyskinesia as the primary risk/concern with increased/long term Reglan use (risk of tardive dyskinesia increases with long
term use and higher dosage), but I wanted to provide you with further information on the handout as you had asked those questions.

There was an addendum done to the visit notes to correct the mistakes. We are so sorry for the errors and thank you for bringing this to our attention. I will publish that to your patient portal. Please let me know if we can be of any further assistance.
Thank you,

Sincerely,

I appreciate the addendum but it's still not correct. The soap note clearly states that the negative breath test for SIBO was on that visit – March 13, 2023 – not the prior visit in December. The addendum only mentions not doing the breath test in December.

Also, I never received any handout at the visit, nor afterwards. The only information I got regarding risk factors for metoclopramide are on the soap note which specifies that we discussed multiple risk factors during the visit. The only risk factor discussed during the visit was tardive dyskinesia.

Please fix the addendum so that it is accurate.

Thank you,
Tammy Stone

I got no reply.  I sent another message a couple of hours ago asking for a signed addendum with the correct information.  As of right now, no reply.

Okay first of all, as to the physical exam.  She stated that it was all visual and verbal. Isn't that what a history is?  That's the history part of the history and physical. Not the physical.  How is a physical exam visual (from 5 feet away) and verbal?

Secondly, she stated that nothing was documented that she couldn't visually assess.  How could she see that my sclera wasn't jaundiced or that my conjunctiva was pink or that my pupils were equal from 5 feet away?  And how could she tell that my neck was supple and there were no masses and that my trachea was midline from 5 feet away?  But that's not even what irked me.

She specifically stated that she performed a breath test for SIBO which came back negative.  Blatant lie.  No such test was performed in either of my visits in December 2022 or March 2023.  And another blatant lie was that she discussed the risks of metoclopramide with me.  She did tell me about tardive dyskinesia.  And I asked what some of the other risk factors were and she said that was basically it, just tardive dyskinesia.  Then in the note she lists all the other risk factors that she said she discussed with me.  Then, in the addendum she has the audacity to state that she gave me a hand out of the risk factors.  Blatant lie.  I was not given any handout regarding metoclopramide risk factors at the visit, nor afterwards in my patient portal documents.

And, if you notice she didn't even sign the addendum.  So, it's not even recorded.  She just sent it to me to appease me.  And it wasn't even correct!

I'm fine with people making mistakes.  Mistyping something or even overlooking something that's automatically entered and not fixing it are excusable mistakes.  What I'm not okay with are blatant lies.

Stating that you performed a breath test – which was never administered – is a blatant lie.  Or stating that you discussed a multitude of risk factors and listing them individually– which only one was discussed – is a blatant lie.  Then stating in the addendum that a handout of said risk factors was given to me–when no such handout was given at the visit nor afterwards – is a blatant lie.

Why lie on medical records?  If you wanted to say that I had a negative breath test, then why didn't you perform one?  If you want to say you discussed risk factors for metoclopramide with me then why did you only mentioned one during our visit, even after being asked specifically what the other risk factors would be?  Because she didn't know any others.  That's why.  She had to look it up and then put it in the soap note as if she discussed it with me.  Then when I called her out on it, she claims she gave me a hand out of the risk factors, when she didn't.  Blatant lie.  And if you want to claim you did a physical exam, could you not at least feel the neck and look in the eyes?

I despise dishonesty.  And it appears as though she's not going to fix the addendum nor sign it.

Word to the wise – do not do business with Shayla Fawcett (physician assistant) at Washington gastroenterology.  She's a terrible medical provider who lies on medical records. These were not simple mistakes.  And whose misdiagnosis caused me to be much worse off than if I had never been treated by her.  And I sure wish I hadn't been..

Also, do not do business with Cascade Eye and Skin at the same Sunrise location in Puyallup Washington.  They are tyrants who discriminate against handicapped people.

That's my two cents.

By Degrees

One constant in life is that things always change.  And like the law of entropy, if we are not constantly working towards order, good and righteousness, it's easy to fall into a state of evil, disarray and disorder.

Many people tend to think that they are the exception to the rule.  But there's one thing about laws of nature, and laws in general – they apply to everyone.

There are a couple of scriptures in The Book of Mormon that mention things changing "by degrees".  This can be very dangerous.  When things change gradually, people tend to readily accept it or not notice the change.

The first scripture that comes to mind is when Lehonti was poisoned "by degrees" until he died.

Alma 47:18

"And it came to pass that Amalickiah caused that one of his servants should administer poison by degrees to Lehonti, that he died."

The story of the frog in the pot gradually being heated until it died comes to mind.  The rising water temperature happened so gradually that the frog didn't notice it until it was too late and he was cooked.

The same thing happens to us.  Small things like watching a movie with "just a couple of bad parts".  People justify it by claiming that it's a good movie, except for… a couple of bad words, a mild sex scene, a little nudity, some violence, etc.  But otherwise, it's a great movie!  No.  If there are bad things in the movie, it's a bad movie.

That's how Satan works.  He throws a little bit of bad in with the good to poison us by degrees.  Why do we allow this?  Why do we watch movies with profanity, sex, violence, etc.?  Why do we rationalize being poisoned by degrees just because most of the movie is good?

Another example is social media.  Why do people follow certain people on social media who post things that poison by degrees?  For instance, worldly influencers who live questionable lifestyles.  Or perhaps friends or family who were once good examples of righteousness and are now apostate and evil.  Most apostates when they leave the church can't leave the church alone.  They must constantly be bashing the church and flaunting their open rebellion. Who needs that poison in their lives?  Certainly not I.

I personally don't like seeing people yield to temptation and fall into Satan's grasp.  But that's what happens when you allow yourself to be poisoned by degrees.  Eventually the gradual poisoning reaches a threshold where you die spiritually.

Often times we care too much about what others' perceptions of us are.  We worry about offending someone by unfollowing them or perhaps having them make fun of us for not wanting to watch a certain movie or TV show.  But what's more important – hurting someone's feelings or your eternal salvation?  Yes, it's that serious.  And honestly, if someone is shallow enough to be offended by you unfollowing them on social media, then they are not worthy of your friendship anyway.

The world we live in has become so evil that even the news has become nothing but the philosophies of Satan being spread as truth.  When it is the complete opposite.  That's why I haven't watched the news in over a decade.  TV shows are the same thing.  I also haven't watched them in decades but on the rare occurrence that I'm stuck watching a TV show for a few minutes waiting for my channel to be turned, I am completely disgusted with the pure evil that is allowed on TV and that people invite into their homes. They are literally allowing their minds and spirits to be poisoned by degrees.

If it were physical poison, instead of spiritual poison, people would avoid it like the plague.  But because it poisons your spirit instead of your body, people not only accept it, they seek it.  That's why so many strong people are dropping like flies.  They are being poisoned by degrees.

On a brighter note though, it works the other way as well.  Instead of being poisoned by degrees, we can prosper by degrees.

Mosiah 21:16

"And it came to pass that they began to prosper by degrees in the land, and began to raise grain more abundantly, and flocks, and herds, that they did not suffer with hunger."

When we humble ourselves and strive to do what is right, we prosper by degrees.  The Lord blesses us.  If we are persistent, we will grow in our prosperity and spirituality.

Again, because it is gradual,we may not notice much of a difference in our everyday lives.  But prospering by degrees is a daily, drop by drop occurrence.  Just like the five wise virgins who accumulated oil for their lamps "by degrees" drop by drop on a daily basis.  They were ready to meet the bridegroom when he came because of their daily effort to fill their lamps with oil "by degrees".

People don't usually change – either for the good or the bad – drastically or suddenly.  But by degrees.  It's what we do on a daily basis that determines who we are.  Our daily choices determine our destiny, by degrees.

Every good thing we do every day is a drop of oil added to our lamps.  Do we choose to fill our lamps with oil, by degrees, and prosper?  Or are we more concerned with the ways of the world?  Do we choose drops of poison instead of drops of oil every day?  Do we choose to be poisoned by degrees rather than prosper?

May we all choose to prosper by degrees rather than be poisoned by degrees.

That's my two cents.

Sunday School Lesson – Miracles

We don't have church till 1:00 PM unfortunately.  We got ripped off because we were supposed to have 11 o'clock church this year.  So, we haven't taught the lesson yet but here's what I prepared:

Tammy’s Sunday School lesson– Miracles –Matthew 8, Mark 2-4, Luke 7

March 5, 2023

From the manual:

One of the clearest messages in the New Testament is that Jesus Christ is a healer. Accounts of the Savior healing the sick and afflicted are many—from a woman with a fever to a widow’s son who had died.

Why the emphasis on physical healing?

What messages might there be for us in these miracles?

Certainly one obvious message is that Jesus Christ is the Son of God, with power over all things, including our physical pains and imperfections. But another meaning is found in His words to the skeptical scribes: “That ye may know that the Son of man hath power on earth to forgive sins” (Mark 2:10). So when you read about a blind person or a leper being healed, you might think of the healing—both spiritual and physical—that you can receive from the Savior and hear Him say to you, “Thy faith hath saved thee” (Luke 7:50).

What are some of the stories of Jesus healing the sick?

·         A leper (Matthew 8:1–4)

·         A centurion’s servant (Matthew 8:5–13Luke 7:1–10)

·         Peter’s mother-in-law (Matthew 8:14–15)

·         A man sick with palsy (Mark 2:1–12)

·         A man with a withered hand (Mark 3:1–5)

·         The son of the widow of Nain (Luke 7:11–16)

Healing a leper

Matthew 8:1–4

When he was come down from the mountain, great multitudes followed him.

And, behold, there came a leper and worshipped him, saying, Lord, if thou wilt, thou canst make me clean.

And Jesus put forth his hand, and touched him, saying, I will; be thou clean. And immediately his leprosy was cleansed.

And Jesus saith unto him, See thou tell no man; but go thy way, shew thyself to the priest, and offer the gift that Moses commanded, for a testimony unto them.

Why would Jesus tell the man that he healed not to tell anyone?

Centurion’s servant

Matthew 8:5-13, see also Luke 7:1-10

¶ And when Jesus was entered into Capernaum, there came unto him a centurion, beseeching him,

And saying, Lord, my aservant lieth at home sick of the bpalsy, grievously tormented.

And Jesus saith unto him, I will come and heal him.

The centurion answered and said, Lord, I am not aworthy that thou shouldest come under my roof: but speak the word only, and my servant shall be bhealed.

For I am a man under aauthority, having soldiers under me: and I say to this man, Go, and he goeth; and to another, Come, and he cometh; and to my servant, Do this, and he doeth it.

10 aWhen Jesus heard it, he marvelled, and said to them that followed, Verily I say unto you, I have not found so great bfaith, no, not in Israel.

11 And I say unto you, That amany shall come from the east and west, and shall sit down with Abraham, and Isaac, and Jacob, in the kingdom of heaven.

12 But the achildren of the kingdom shall be cast out into bouter darkness: there shall be cweeping and gnashing of teeth.

13 And Jesus said unto the centurion, Go thy way; and as thou hast believed, so be it done unto thee. And his servant was healed in the selfsame hour.

What was it about the centurion’s faith that impressed Jesus?

How can we show similar faith in Jesus Christ?

Peter’s mother-in-law

Matthew 8:14–15

14 ¶ And when Jesus was come into Peter’s house, he saw his wife’s mother laid, and sick of a fever.

15 And he touched her hand, and the fever left her: and she arose, and ministered unto them.

What does she do as soon as she is healed?

 A man sick with palsy

Mark CHAPTER 2

Jesus forgives sins, heals a paralytic, eats with tax gatherers and sinners, and announces that He is Lord of the Sabbath.

And again he entered into Capernaum after some days; and it was noised that he was ain the house.

And straightway many were gathered together, insomuch that there was no room to receive them, no, not so much as about the door: and he apreached the word unto them.

And they come unto him, bringing one sick of the palsy, which was aborne of four.

And when they could not come nigh unto him for the press, they uncovered the roof where he was: and when they had broken it up, they let down the bed wherein the sick of the palsy lay.

When Jesus saw their faith, he said unto the sick of the palsy, Son, thy sins be aforgiven thee.

But there were certain of the scribes sitting there, and reasoning in their hearts,

Why doth this man thus speak ablasphemies? who can forgive sins but God only?

And immediately when Jesus perceived in his spirit that they so reasoned within themselves, he said unto them, Why reason ye these things in your hearts?

Whether is it easier to say to the sick of the palsy, Thy sins be forgiven thee; or to say, Arise, and take up thy bed, and walk?

10 But that ye may know that the aSon of man hath power on earth to forgive sins, (he saith to the sick of the palsy,)

11 I say unto thee, aArise, and take up thy bbed, and go thy way into thine house.

12 And immediately he arose, took up the bed, and went forth before them all; insomuch that they were all amazed, and glorified God, saying, We never saw it on this fashion.

The account of the miracle in Mark 2:1–12 teaches, among other things, the value of working together in unity to assist the Savior in ministering to those in spiritual or physical need.

What can we learn from Mark 2:1–12 about the value of working in unity to serve those in need? (see also Mark 3:24–25).

A man with a withered hand

Mark 3:1–5

And he entered again into the synagogue; and there was a man there which had a withered hand.

And they watched him, whether he would heal him on the sabbath day; that they might accuse him.

And he saith unto the man which had the withered hand, Stand forth.

And he saith unto them, Is it lawful to do good on the sabbath days, or to do evil? to save life, or to kill? But they held their peace.

And when he had looked round about on them with anger, being grieved for the hardness of their hearts, he saith unto the man, Stretch forth thine hand. And he stretched it out: and his hand was restored whole as the other.

(Healing on the Sabbath)

The son of the widow of Nain

11 ¶ And it came to pass the day after, that he went into a city called Nain; and many of his disciples went with him, and much people.

12 Now when he came nigh to the gate of the city, behold, there was a dead man carried out, the only son of his mother, and she was a widow: and much people of the city was with her.

13 And when the Lord saw her, he had acompassion on her, and said unto her, Weep not.

14 And he came and touched the bier: and they that bare him stood still. And he said, Young man, I say unto thee, Arise.

15 And he that was adead sat up, and began to speak. And he delivered him to his mother.

16 And there came a afear on all: and they glorified God, saying, That a great prophet is risen up among us; and, That God hath bvisited his people.

Calming the Tempest

Mark 4:35-41

35 And the same day, when the even was come, he saith unto them, Let us pass over unto the other side.

36 And when they had sent away the multitude, they took him even as he was in the ship. And there were also with him other little ships.

37 And there arose a great storm of wind, and the waves beat into the ship, so that it was now full.

38 And he was in the hinder part of the ship, asleep on a pillow: and they awake him, and say unto him, Master, carest thou not that we perish?

39 And he arose, and rebuked the wind, and said unto the sea, aPeace, be still. And the wind ceased, and there was a great bcalm.

40 And he said unto them, Why are ye so afearful? how is it that ye have no bfaith?

41 And they feared exceedingly, and said one to another, What manner of man is this, that even the wind and the sea obey him?

Have you ever felt the way Jesus’s disciples did in the storm at sea—watching the waves of water fill the boat and questioning, “Master, carest thou not that we perish?”

 Activity:

In Mark 4:35–41, you will find four questions. List each one, and ponder what it teaches you about facing life’s challenges with faith in Jesus Christ.

How does the Savior bring peace to the storms of your life?

Write on one side of the paper a trial/trials you have experienced.

On the other side, write something from -Mark 4:35-41 that inspires you to turn to the Savior during your trials.  In other words, write how Jesus helps you through your trials.

Would anyone like to share what you wrote?

Master, the Tempest Is Raging

Fervently

1.Master, the tempest is raging!

The billows are tossing high!

The sky is o’ershadowed with blackness.

No shelter or help is nigh.

Carest thou not that we perish?

How canst thou lie asleep

When each moment so madly is threat’ning

A grave in the angry deep?

The winds and the waves shall obey thy will:

Peace, be still.

Whether the wrath of the storm-tossed sea

Or demons or men or whatever it be,

No waters can swallow the ship where lies

The Master of ocean and earth and skies.

They all shall sweetly obey thy will:

Peace, be still; peace, be still.

They all shall sweetly obey thy will:

Peace, peace, be still.

2.Master, with anguish of spirit

I bow in my grief today.

The depths of my sad heart are troubled.

Oh, waken and save, I pray!

Torrents of sin and of anguish

Sweep o’er my sinking soul,

And I perish! I perish! dear Master.

Oh, hasten and take control!

3.Master, the terror is over.

The elements sweetly rest.

Earth’s sun in the calm lake is mirrored,

And heaven’s within my breast.

Linger, O blessed Redeemer!

Leave me alone no more,

And with joy I shall make the blest harbor

And rest on the blissful shore.
